The Dale House Project seeks a Transition Specialist Coordinator . The Dale House is a “Christ-centered community of and for broken people doing life together.” 

We offer several services where young people learn how to live on their own independently.  Our clients are typically facing homelessness or unstable housing, paroling from Colorado Division of Youth Services, or transitioning out of Colorado Department of Human services (foster care). 

As the Transition Coordinator, you oversee the Transition Team ensuring that the team meets organizational goals, adheres to policies and standards, and delivers high-quality services to clients. This position acts as a bridge between the specialists and leadership, ensuring smooth communication, effective workflow, and the professional development of the team. planning, implementation, and scheduling of programs. You take hold of several transition/case management processes, ensuring that our program stays organized, maintaining accountability for deadlines and services to clients, and being available for the transition team. Your role ensures proper communication is happening between coordinators of other programs to provide a holistic care approach to youth. You also will take the opportunity provided to serve as a mentor, leader, and companion to our staff as they go through a very difficult learning process in Christian ministry among at-risk youth. This position’s schedule requires the ability to work in person, some nights (specifically Wednesday nights) and attend scheduled meetings. The position requires an individual who can communicate effectively about their working hours and have flexibility. 

This position is a great fit for a person who enjoys creating structure, managing teams through delegation, who works patiently with people who are figuring out the expectations of a professional work environment, and excels in efficient communication between programs.  It also allows those organizational gifts to meet the needs of an organization that, for 50 years, has shown Christ-like love for teenagers who have experienced a great deal of brokenness and staff who yearn to grow closer to Jesus in relational ministry.

The Transition Coordinator  job has several key components: 

  • Managing Transition Team. Our transition specialists are responsible for several case needs of all of our clients. They are responsible for treatment plans, 1on1’s, creating and facilitating development groups, case management needs such as Food stamps, housing assistance, Medicaid, health appointments, connection to school, employment, etc. They are looked to by staff for updates on client’s goals and connection to mental health. Your role is essential in assistance and support to ensure they are meeting these deadlines and tracking their caseload(s). 
  • Providing quality care to youth. You will be responsible for having a caseload of your own to also provide these services to youth firsthand. Additionally, you will be responsible for attending all youth team support meetings to provide updates and communicate effectively with other staff. 
  • Communication between programs. As a coordinator it will be vital to communicate with other coordinators to ensure we are working together as staff to provide holistic care. This will include working with the Housing and Employment coordinator in scheduling development groups that align with what client’s needs are and are made available in times that work best for youth. This will include working with the Daytime Coordinator to discuss groups offered throughout the day as well.

Who You’d Be Working With:

You will report to the Clinical Director, but you're also part of a team that includes our Program Coordinators (who are responsible for our youth programs; Daytime, Employment, and Housing), and Quality Assurance who ensures staff are meeting documentation and HIPAA standards. You will also work closely with the Co-Executive Director of Training and Discipleship to ensure all training follows the mission and values of the Dale House Project.  

The ideal candidate will possess certain essential traits for success in this role:

  • Authentic Faith. Has a heart for Jesus and seeks to live out their faith more deeply.  Often, that means exploring whether personal sacrifices are necessary to walk by faith.
  • Heart for Young Adults and Youth. Has compassion for youth who have faced difficulty in their lives.  Seeks to make connections with people who have very different life experiences - juvenile justice, homelessness, involvement in foster care, etc.
  • Task Oriented. Has, or can create, a system for keeping tasks from slipping through the cracks. Ability to manage time effectively. Prioritize and manage multiple tasks and caseloads simultaneously so clients are getting their needs met. Ensure the Transition team is meeting deadlines without compromising quality.
  • Leadership/Collaboration/Communicator. Communicates and manages a team of people well to keep everyone on the same page, model how to provide Transition services, and provide support in difficult situations with clients. Lead initiatives to implement innovative solutions and manage communication of changes within the organization. Ability to observe groups and 1on1’s providing quality feedback and support to the team. 
  • Adaptability / Flexibility.  Every day at the Dale House is full of surprises, so this candidate can roll with new information, changing goals, and unexpected hiccups.  Can juggle competing demands and prioritize based on new information. Can problem solve and remain calm under pressure. Is open to filling in when clients have unexpected needs and a Transition Specialist is busy. 
  • Organized/Discipline/Consistent. Consistent ability to adhere to policies and procedures. Strong work ethic with a commitment to maintaining high standards. Proven ability to organize and manage multiple priorities. Strong attention to detail and accuracy. Ability to create and follow structured plans and schedules.

Compensation:

This is a full-time position. Salary Range: $45,000 to $50,000, with a full benefit package.

Dale House covers the cost for health coverage (monthly premiums) at no expense to you. Other benefits come from Young Life, including: dental, vision, basic life insurance, and a match on 401(k) contributions up to 4%.
